Below are a variety of important forms that you may need to access and complete throughout the school year.

Emergency Medical  Required for registration - update annually and with changes.
Health History  Required for registration.

Registration

  
Registration 2023-2024  Required to enroll new student for the new school year.
Re-Registration 2023-2024  Required to re-enroll students for the new school year.
Tuition Contract   Required to re-enroll students for the new school year. 
Parishioner Verification Letter   Required for all families that belong to another parish. Included on the registration forms.

Photos / Internet

  
Internet Policy & Agreement  All parents and students must read annually
Photo/Publication Permission  Parents sign annually to grant/restrict photo and publication use 

Communication

  
Email Authorization  Parents sign to authorize school to contact you by email
Volunteer Sheet To be completed by parents/guardians.
Student Data Sheet To be completed by parents for each child. Please fill in all information.
Parent/Student Contract To be signed by parents and students after reviewing Handbook and Parental support statement

To be reviewed annually by parents
