| Registration 2024-2025 | Required to enroll new student for the new school year. |
| Re-Registration 2024-2025 | Required to re-enroll students for the new school year. |
| Tuition Contract | Required to re-enroll students for the new school year. |
| Parishioner Verification Letter | Required for all families that belong to another parish. Included on the registration forms. |

| Internet Policy & Agreement | All parents and students must read annually |
| Photo/Publication Permission | Parents sign annually to grant/restrict photo and publication use |

| Email Authorization | Parents sign to authorize school to contact you by email |
| Volunteer Sheet | To be completed by parents/guardians. |
| Student Data Sheet | To be completed by parents for each child. Please fill in all information. |
| Parent/Student Contract | To be signed by parents and students after reviewing Handbook and Parental support statement |
| Parental Support Statement | To be reviewed annually by parents |
